Fall 24 Workshop- Monster Mash-Up

My child and I understand the importance of respectful behavior. We also understand that he/she may be dismissed from the program for poor behavior. Notification of a problem will be made to me by phone before dismissal. I agree to allow my child to be photographed or videotaped for publicity purposes for Starting Arts. I hereby release, waive, discharge, indemnify, and hold harmless Starting Arts, its directors, officers, employees and agents, from and against any claim for damage, injury, loss or death to the registered student resulting from participation in any class, program, production or other activity either at Starting Arts or at another location.
