My child and I understand the importance of respectful behavior and regular class attendance. We also understand that he/she may be dismissed from the program for two unexcused tardies/absences, or three documented incidents of poor behavior. Notification of a problem will be made to me by phone/mail after the second incident. I agree to allow my child to be photographed or videotaped for publicity purposes for Starting Arts. I hereby release, waive, discharge, indemnify, and hold harmless Starting Arts, its directors, officers, employees and agents, from and against any claim for damage, injury, loss or death to the registered student resulting from participation in any class, program, production or other activity either at Starting Arts or at another location.

I have read and agree to the terms on the Starting Arts' Performer Behavior Contract located on the prior event information page.

Refunds must be requested prior to the start date to be processed. No refunds will be processed once the program begins.

Please be aware that due to the nature of theater, our staff may sometimes need to assist children with their costumes, wigs, and tech equipment, which might involve inadvertent touch. All Starting Arts staff have gone through a live scan/fingerprinting process through the DOJ before being hired. Our policy ensures that there are always two mandated reporter staff within sight of the children at all times, prioritizing their safety and comfort above all else.
